I wouldn't really consider Azagthoth and Rutan a duo.
As far as true duos go I'll have to go with Tipton and Downing. Mustaine and Friedman were a damn good duo but it didn't last long enough.

Some bands (CC, MA, etc) had several different line ups and some where there for those bands more popular albums while others are there for longer periods of time while others may have been there for more influential albums. Not all of those choices were easy.
